APPOINTMENTS

 ·         Please call 760-724-8782 to make an office appointment.

 ·         Our office hours are from 8:30AM to 4:30PM, Monday through Friday. 

 ·         Please arrive at the office 15 minutes prior to your scheduled appointment to complete the check-in process.

 ·         The appointment is for consultation with one of our physicians.  You will not undergo any procedures on this day. If a procedure is needed, it will be scheduled for a different day.

  ·         We have a backlog of patients needing appointments. Thus we believe it is not fair to other patients when an appointment time goes unused.  If we receive less than 48 hours notice of cancellation there will be a $35.00 charge.

 BRING THE FOLLOWING WITH YOU TO YOUR APPOINTMENT.

1.       Fill out and bring the registration forms to your appointment.  You can obtain these in several different ways:

1. Download the forms by clicking the link here, Patient Forms

2. We can e-mail them to you.

3. We can mail them to you.

2.       List of current medications.

3.       Pertinent records from other doctor visits that you may keep at home.

4.       Results of any prior procedures (colonoscopy, EGD, etc.).

5.       Any X-ray, CT, MRI reports.

6.       Any blood tests results.

7.       Current Insurance Card.

8.       Amount of co-pay if required by your insurance.

9.       Interpreter if needed. Bring an English-speaking family member or friend.
